• Conducting comprehensive assessments to identify the needs and strengths of individuals with learning disabilities.
• Developing, implementing, and reviewing person-centered care plans in collaboration with clients, families, and other professionals.
• Providing support and guidance to clients and their families, promoting independence and social inclusion.
• Maintaining accurate records and documentation in compliance with local policies and regulations.
• Participating in team meetings and case conferences to share insights and strategies for effective service delivery.

To be considered for this position, you must possess the following essential qualifications and skills:

• A recognized social work qualification (e.g., BA/MA in Social Work).
• Current registration with Social Work England.
• Experience working with adults with learning disabilities or in a similar social work setting.
• Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues.
• Knowledge of relevant legislation and best practices in social work.
