Qualified Social Worker - Review & Extra Care Team £30 per hour | Hybrid Working | Salford Adult Health & Social Care, 1 City Approach, Albert Street, Eccles, Manchester, M30 0BL
Eden Brown Synergy are currently recruiting for a Qualified Social Worker to join the Review & Extra Care Team within Adult Health & Social Care at Salford City Council.
This is an excellent opportunity to work within a dynamic adult services team, focusing on reviews, extra care housing, and promoting independence through person-centred practice.
Key Responsibilities:Completing statutory Care Act reviews
Supporting individuals living within Extra Care Housing settings
Undertaking strength-based assessments and support planning
Managing a caseload of adults with varying levels of need
Working collaboratively with health professionals and partner agencies
Ensuring safeguarding responsibilities are fulfilled in line with statutory duties
Essential Requirements:Qualified Social Worker
Registered with Social Work England
Experience within Adult Social Care
Knowledge of Care Act legislation and review processes
Ability to work effectively within a hybrid model
