Your key responsibilities will include:
Conducting comprehensive assessments to determine the needs of clients post-discharge.
Collaborating with healthcare professionals, families, and community resources to facilitate smooth transitions.

Providing ongoing support and monitoring to clients and their families during the transition process.
Maintaining accurate and up-to-date case records in compliance with local policies.
To be considered for this position, you must possess the following qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work or equivalent).
Registered with Social Work England.
Proven experience in a social work role, particularly in discharge planning or hospital settings.
Strong interpersonal and communication skills, with the ability to work collaboratively in a multidisciplinary team.
