About the Role
A Social Worker is required for work within the Discharge To Assess Team in the vibrant city of Bristol. Excellent rates of pay up to £32ph. 3 month minimum contract but likely extended. 37 hours per week. Exciting opportunity in a great city with good transport links to neighbouring authorities. Hospital based social work role. Based in the office 5 x days per week
Key job outcomes/accountabilities
1. To carry out the statutory duties and responsibilities of Bristol City Council within relevant legislative frameworks.
2. To practice in accordance with Social Work England (SWE), professional capabilities framework (PCF) and Knowledge and skills statement (KSS) for social workers in adult services.
3. Manage a workload commensurate with level of competence, qualification as an experienced social worker, within eligibility criteria, prioritisation of service delivery and available resource.
4. To enable self-directed support with people and carers:
* by undertaking assessments to identify the outcomes and the personal budget for the provision of services,
* by developing and reviewing support plans and provision against outcomes and changing needs.
5. To undertake risk assessments and be responsible for actions in situations where a person's safety or liberty is at risk, observing safeguarding adults' policies and procedures, the legal principles of the Mental Capacity Act 2005 and Care Act 2014 and the principles of positive risk taking.
6. To advise on and enable the creative use of universal and community resources, including tech enabled support and other equipment.
7. To contribute to the development and supervision of social care workers. To undertake student supervision.
8. To concentrate on specific areas of work requiring more developed skills, i.e. promoting best practice in use of the Mental Capacity Act 2005, the Mental Health (amendment) Act 2008 and Care Act 2014 safeguarding adults policies and procedures.
9. To ensure that information shared with people who draw on care and support is in an appropriate format. To ensure information shared with other professionals is in accordance with the Data Protection Act 1998, GDPR and other information sharing protocols, including the safeguarding adult's information sharing protocol.
10. Apply an advanced knowledge of a wide range of legislation, policies and evidence informed practice.
11. Undertake an additional role including Approved Mental Health Professional, Best Interest Assessor, Practice Assessor, or another role as agreed by the career progression panel. You will be expected to take on the additional duties associated with the role including, active participation on a rota or having a social work student along with undertaking ongoing CPD to maintain your currency to practice in chosen specialism.
You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK
