Job Description
  • Administer skilled nursing care for clients of all ages in their place of residence.
  • Assess home health and hospice patients to identify physical, psychosocial, and environmental needs.
  • Implement, develop, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
  • Provide education to patients and their families regarding care, diet, medications, and home safety.
  • Supervise and provide clinical direction to home health aides to ensure service quality.
  • Monitor assigned cases to ensure compliance with third-party payer requirements.
  • Perform on-call responsibilities and provide on-call services as needed.
Critical Information
  • On-call required if requested; on-call needs are subject to change based on staff and branch needs.
Education/Licenses Needed
  • Current state license as a Registered Nurse.
  • Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
  • CHPN certification preferred.
  • Proof of current CPR certification.
