JOB REQUIREMENTS
- Board: NP or PA certification required
- Licenses: Active PA license required
- DEA: Active DEA required
- Certifications: ACLS, BLS
- All required documentation must be submitted no later than 5 business days prior to start
- Provider questionnaire required at time of submission
- NPDB self-query required at time of presentation, within 6 months
- Full disclosure on all malpractice required
- If the role includes specific coverage dates, your submission must clearly state exact dates the provider can work
- Submissions without rates will be rejected immediately
- Required Submission Packet: CV, Coversheet, Questionnaire, NPDB (pulled within the last 6 months)
- Provide clinical support within the Interventional/Electrophysiology department
- Manage patient care for a population aged 18-100
- Collaborate with the assigned clinical team to ensure 40 hours of coverage per week
- Monitor patients postoperatively following interventional and electrophysiology procedures
- Maintain accurate documentation within the EPIC EMR system
