The Role We are currently recruiting an experienced Social Worker to join the Adults Locality Team within Somerset Council. This is an exciting opportunity to join a well-structured, supportive and fast-paced team that is expanding its workforce.

Key Responsibilities

  • Work closely with professionals across health, voluntary and statutory sectors as part of a multi-disciplinary team, ensuring effective communication and shared best practice.
  • Review, monitor and evaluate care plans and the quality of service provision, responding proactively to changing needs.
  • Develop, implement and assess reablement programmes within care packages, promoting independence and positive outcomes.
  • Manage your own caseload or specialist projects, ensuring appropriate interventions and timely outcomes.
  • Complete risk assessments, safe methods of work practice guides, and raise awareness of a wide range of care-related issues and their implications.
  • Undertake visits across the community, including home visits, as required.
  • Qualified Social Worker with relevant experience in Adult Social Care
  • Full UK driving licence and access to a vehicle (or ability to travel via other suitable means)
  • Ability to work flexibly, with the role being predominantly office and community-based, alongside some home working
