About the Role

<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">Conducting initial assessments and reassessments for members/clients for the elderly and disabled waiver program.<o:p></o:p></span></p>
<p><b><u><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">KEY RESPONSIBILITIES:<o:p></o:p></span></u></b></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">1.<span style="mso-spacerun: yes;"> </span>Completes initial and annual assessments in client/member’s home and determines member ability to complete IADLs and ADLs.<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">2.<span style="mso-spacerun: yes;"> </span>Completes modified assessments as needed for transfers, hospital and nursing home<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">discharges, and increased or added services.<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">3.<span style="mso-spacerun: yes;"> </span>Assures members have services to remain in their community and fulfills the fiduciary obligation to meet members’ needs based on guidelines.<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">4.<span style="mso-spacerun: yes;"> </span>Communicates monthly with administrator to receive re-assessment schedule for SOURCE<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">and CCSP annual review.<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">5.<span style="mso-spacerun: yes;"> </span>Review medications for appropriateness. Makes note of any new medications, dosages, and discontinued medications.<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">6.<span style="mso-spacerun: yes;"> </span>Documents assessment entries into Harmony system within guidelines established by the<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">agency and/or regulatory groups. Requests medical and hospital records as needed to assist with determining eligibility. <o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">7.<span style="mso-spacerun: yes;"> </span>Participates in team meetings with Medical Director weekly to present all approved new admits, reassessments, hospitalizations, Home Health admits and discharges, and repeat variances. Also, members with potential for discharge, actual discharges, and situations threatening someone’s health or functional status.<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">8.<span style="mso-spacerun: yes;"> </span>Attends and participates in mandatory in-services.<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">9.<span style="mso-spacerun: yes;"> </span>Complies with corporate compliance program.<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">10. Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">11. Follows established safety regulations, to include fire protection & prevention, smoking<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">regulations, and infection control and prevention.<o:p></o:p></span></p>
<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">12. Follow established safety procedures when performing tasks and/or working with equipment.<o:p></o:p></span></p>

<p><span style="font-size: 10.5pt; font-family: 'Arial',sans-serif;">•<span style="mso-spacerun: yes;"> </span>Visits clients/members in their residence to provide care or services according to rules and regulations for elderly and disabled waiver program guidelines.<o:p></o:p></span></p>
