    <p>The Licensed Clinical Social Worker collaborates with the multi-disciplinary team to provide input in the development of the plan of care for those patients/clients requiring social work intervention. The Licensed Clinical Social Worker provides counseling and crisis intervention to patients/clients who have social or emotional problems in a variety of settings across the age span.</p>
    <p>Minimum Requirements:</p>
    <ul>
      <li>Master’s degree in Social Work from a school of social work accredited by the Council on Social Work Education required</li>
      <li>Active license as an LCSW in state of practice required</li>
      <li>Minimum of one year of social work experience preferred</li>
      <li>Current CPR if applicable</li>
      <li>TB questionnaire, PPD or chest x-ray if applicable</li>
      <li>Current Health certificate (per contract or state regulation)</li>
      <li>Must meet all federal, state and local requirements</li>
      <li>Must be at least 18 years of age</li>
