Are you passionate about connecting people with resources and services that improve lives? We are seeking a dynamic and relationship-focused Outreach Coordinator to join our Extended Case Management (ECM) team. In this role, you' ll help expand access to critical support services by building partnerships, conducting community outreach, and connecting eligible individuals to programs that meet their needs.

What You' ll Do
Community Outreach & Client Recruitment
Increase program awareness through community events, workshops, presentations, tabling events, and informational sessions.
Build referral pipelines that connect eligible individuals to available services.
Serve as a point of contact for prospective clients, providing program information and enrollment support.
Follow up with referrals and interested participants to encourage successful enrollment.
Partnership Development & Community Engagement
Build and maintain strong relationships with community organizations, healthcare providers, public agencies, faith-based groups, and service providers.
Attend networking events, coalition meetings, and community gatherings throughout the Sacramento region.
Collaborate with internal teams to strengthen referral partnerships and expand program visibility.
Outreach Materials & Communications
Develop and distribute culturally responsive outreach and educational materials.
Assist with community communications, social media outreach, and informational campaigns.
Enrollment Strategy & Pipeline Management
Focus outreach efforts on programs with enrollment needs.
Monitor referral and enrollment trends and adjust outreach strategies accordingly.
Identify barriers to enrollment and recommend solutions to improve participation.
Data Tracking & Reporting
Maintain accurate records of outreach activities, referrals, partnerships, and enrollments.
Utilize databases and tracking systems to monitor performance metrics.
Support reporting requirements and provide insights on outreach effectiveness.
Qualifications
Required
Bilingual in Ukrainian or Russian
Strong verbal and written communication skills
Ability to build and maintain professional relationships with diverse stakeholders
Excellent customer service and organizational skills
Self-motivated with strong problem-solving abilities
Ability to work effectively with people from diverse cultural backgrounds
Comfortable conducting presentations and community outreach activities
Availability for occasional evening and weekend events
Preferred
Experience in community outreach, social services, nonprofit organizations, public health, case management, or community engagement
Experience developing partnerships and referral networks
Knowledge of community resources and service systems
Experience with CRM or case management databases
