The RN Case Manager serves as a patient advocate and works collaboratively with physicians, nurses, social workers, therapists, payers, patients, and families to develop individualized care plans, coordinate services, and facilitate timely discharge planning.

Position Details

  • Position: Registered Nurse (RN) Case Manager
  • Department: Case Management / Care Coordination
  • Facility Type: Acute Care Hospital

Core Responsibilities

The RN Case Manager will:

  • Perform comprehensive assessments of patients’ clinical, functional, psychosocial, and discharge needs
  • Coordinate individualized care plans in collaboration with physicians and interdisciplinary teams
  • Facilitate safe and timely patient discharges and transitions of care
  • Collaborate with patients, families, caregivers, providers, and community resources to address barriers to discharge
  • Participate in interdisciplinary rounds and communicate patient progress and care goals
  • Review medical records to support appropriate level of care and medical necessity
  • Perform utilization review activities and collaborate with insurance providers as needed
  • Identify opportunities to improve patient outcomes, reduce delays, and promote efficient resource utilization
  • Assist with arranging post-acute services, including home health, rehabilitation, skilled nursing facilities, and other community resources
  • Provide education and support to patients and families regarding care plans and discharge expectations
  • Maintain accurate documentation according to hospital policies and regulatory standards

Successful candidates will demonstrate:

  • Strong clinical assessment and critical thinking skills
  • Excellent communication and collaboration abilities
  • Knowledge of acute care workflows and discharge planning processes
  • Ability to manage multiple patient cases and prioritize competing needs
  • Understanding of utilization management principles and healthcare reimbursement processes
  • Strong organizational skills and attention to detail
  • Active Registered Nurse (RN) license
  • Associate Degree in Nursing (ADN) required; BSN preferred
  • Previous acute care nursing experience required
  • Case Management, Utilization Review, or Discharge Planning experience preferred
  • BLS certification required
  • Case Management certification (CCM) preferred but not required
