Manage benefits, oversee services, act as a case manager and trainer to achieve optimal results. Effective care management helps minimize unnecessary resource use.</p><h4>Essential Functions of the Role</h4><ul><li>Conduct thorough assessment, implement care plan, collaborate with team for evidence-based high standard of care.</li><li>Creating patient care plans that focus on interventions, treatment goals, and self-management for the patient's benefit.</li><li>Examine the health needs of patients and families, then provide necessary care to meet those needs.</li><li>Assisting patients with self-management using education, personal visits, and telephone interactions, inspiring patients to stick to their care plans.</li><li>Researching resources for patient needs; coordinating programs for correct service delivery. Providing referrals to help patients.</li><li>Being proactive in identifying problems and solutions in coordinating care to provide the best possible options.</li><li>Acting as a bridge between healthcare providers to ensure smooth communication and coordination among different facilities.</li></ul><h4>Key Success Factors</h4><ul><li>A comprehensive knowledge of care management, along with efficient resource and utilisation management.</li><li>Proficiency in patient assessment and care management.</li><li>Exceptional communication skills, both verbal and written, are crucial.</li><li>The ability to keenly observe, assess, and document patient progress in relation to a care plan.</li><li>The aptitude to assist patients with accessing community resources.</li><li>The willingness and aptitude to work in harmony with both internal medical staff and external partners or organisations.</li><li>Adaptive to change and able to comfortably navigate through a crisis environment.</li><li>Exceptional discretion and respect for confidentiality as it pertains to all patient information, in accordance with policies and regulations.</li></ul><h4>Belonging Statement</h4><p>We believe that all people should feel welcomed, valued and supported.</p><p><b>Qualifications</b></p><ul><li>A minimum of an Associate's degree is required.</li><li>The applicant should have at least two years of professional experience.</li><li>The applicant should be a licensed Registered Nurse.</li></ul>
