Job Description
Provide primary and family evidenced-based care including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients.
Perform administrative duties such as opening/closing the clinic, inventory, patient follow-up, and maintaining a clean environment.
Handle patient appointments and conduct routine examinations to ensure positive health outcomes.
Encourage patient engagement in their healthcare and well-being through services such as health screenings and chronic condition management.
Collaborate with the clinical care team to drive better outcomes for the patient panel.
Critical Information
Working hours: M-F 8:30-7:30, Sat 9-5:30, Sun 9-4:30; Training M-F 40 hrs.
This role is onsite as needed and requires working every other weekend with possible holiday rotations.
Education/Licenses Needed
Master's Degree level Family Nurse Practitioner program with current National Board Certification and state license to practice as an Advanced Practice Nurse.
A current BLS from ARC or AHA.
