What you will do:
Carry out thorough Care Act assessments and reviews for adults requiring support.
Develop personalised Care and Support Plans tailored to individual needs.
Manage a varied adult social care caseload effectively and responsibly.
Address safeguarding and self-neglect cases with professionalism and care.
Work closely with health professionals and partner agencies to deliver holistic support.
